What is Transition?

Moving from one stage to another.

What is the Central Westmoreland Transition Coordinating Council?

A consortium of parents of special needs students, school district representatives, and public agency representatives who wish to inform interested parties about transition services for students as they move from the school setting to adult life.

What is the goal of the transition council?

To develop a partnership of students and families, school-age services and programs, post-school services and programs, and local community agencies that will work together to bridge the gap between school programs and the opportunities in adult life.

How do I make transition meaningful for my child?

  • Be involved!
  • Attend I.E.P. Conferences.
  • Talk to guidance counselors and teachers about your goals for your child beyond high school or age 21.
  • Support your child in seeking future training and part time jobs that expand skills.
  • Encourage your child to be a self-advocate.
  • Take a pro-active role in educational decision-making.
  • Seek information from community agencies.
  • Ask questions.
