Immunization Requirements

According to Pennsylvania State Law, the immunizations listed below, are required for attendance in school. Physician documentation of immunizations are required prior to attendance in school. Parent hand written copies will not be accepted.

A record of immunizations is maintained for each student by the schools nurse. If your child has a medical appointment and receives any immunizations, please ask for a copy of your child’s updated immunization record and send it in to your school nurse. Remember to keep a copy for your home records.
